Dear Swim Parent,

I realize you have other choices when it comes to swim lessons and I would like to thank you for the opportunity to teach your children to swim.

You will be given a packet of information that includes:

-  General Policies

-  Swim Girl Registration
-  Emergency Contact Information
-  Health History Questionnaire

These forms need to be filled out completely and accurately as they are essential to help develop an individualized program that addresses your child's needs, goals, and interests.

Please return all forms to your swimming location/Preschool.

My goal is to place the children according to their classroom.  This will allow their teachers to help prepare them for their lessons all at once and they will also feel more comfortable with the students they know from their existing classrooms at their preschool.

Payment is requested before the first day of each session.  The registration fee is annual and covers an underwater photograph, certificate, Red Cross water safety book and award for each student per session.

Please drop off payment at your swimming location/Preschool.

We will be sending a progress report home after each session to inform you of your child's progress including their strengths and weaknesses as well as any accomplishments.  If you have e-mail, I would be happy to e-mail you this information.

GENERAL POLICIES

  1. Parent will certify that the answers to the questions outlined on the health history are true and complete to the best of his/her knowledge at the time of signing.  Parent must understand and agree that it is his/her responsibility to inform the swim instructor and Swim Girl, Inc. in writing, of any conditions or changes in the child's health, now and on going, that would affect the student's ability to participate safely and/or providing Swim Girl, Inc. with immediate written notice of any conditions and/or changes in the student's health and welfare that would affect the safety of other students or Swim Girl instructors.

  2. The student is not obligated to perform nor participate in any activity that the parent does not wish them to do.  Parent is responsible to notify his instructor if the child feels any lightheadedness, dizzy, nauseated, or experience any pain or discomfort.

  3. If parent and child arrive late, there is no guarantee the child will receive the full session with the swim instructor.  In return if the swim instructor is late or absent (with no substitute), the child will still receive the full session time.

  4. Each swim session is on a pre-pay basis only.  Registration and reservations for a student's swim lesson(s) must be made and paid in advance no later than the first day of scheduled swim lessons.  In the event that a student's registration, reservation is made without payment, the student will be unable to participate in their scheduled swim lesson(s) until payment or arrangements are made.

  5. I understand that Swim Girl, Inc. has a no refund policy.  In the event, following a student's registration, reservation and advance payment, if a student is unable to participate either partially or fully in the scheduled swim lesson(s) due to no fault of Swim Girl, Inc., at its sole discretion, may permit the student to reschedule.

  6. All swim sessions are non-refundable.  If a swim lesson or session is cancelled by Swim Girl, Inc. (i.e.: due to weather or lack of instructor, etc.) make-up lesson(s) will be scheduled. If the scheduled make-up day is unavailable the parent may choose either a later make-up class(es) or a discount coupon on the next session.  All swim session coupons must be redeemed within six months of the date of issue.

  7. All swim lessons are scheduled and thus require that I provide 24-hour notice when canceling a lesson.  Should you cancel a session with less than 24 hours notice, you will be charged for that lesson and will not be able to have a make-up.

  8. Should your child's swim instructor become ill or is off, another swim instructor will be assigned to you child.  In the event that Swim Girl, Inc. no longer employs your swim instructor, a suitable swim instructor will be re-assigned to oversee your child's swim lessons.

CANCELLATION AND MAKE-UP POLICIES

To Swim Girl Clients:

Your swim instructor is very happy to help you and your child reach their swimming potential.  We know how valuable your time is, so we make every effort to be prompt for your child's specific swimming time.  Please keep in mind that we too have a busy schedule and appreciate your promptness.

If an appointment cannot be kept, please call Swim Girl at (520) 219-9060, 24 hours in advance, if possible.  If a swim lesson is missed and no cancellation is made, you will be charged for that swim lesson.

Since we do want your child to be successful in reaching their goals, we do allow a maximum of 2 make-up lessons per session.  Make-up lessons must be accomplished prior to the current session expiration date and make-up lessons will only be granted if the 24 hour cancellation policy was followed.
